Book & DVD Check Out Form

Please use this form in order to check out books or DVDs from the library. 
Your materials will be delivered to your A block class for you.
Materials may be borrowed for up to 2 weeks. 
You will receive an email when borrowed materials are due back to the library. 
Materials will be set aside for one week after being returned before they will be allowed to be checked out again.
